Home Support

The COA Domiciliary Care Team organises help for clients at home and in the centre:
  • Domestic assistance
  • Food preparation
  • Personal care, showering & dressing
  • Carer relief
  • Help with shopping (including Shopping Project)
  • Case planning and case management
  • Counselling and social support
  • Hospital and home visiting
  • Telephone monitoring of the housebound and lonely
  • Comprehensive assessments
  • Help in emergencies
  • Transitioning from hospital to home, or home to residential care
  • Transport to medical appointments
  • Information and referal

Assessment
All people requesting COA services are visited at home and assessed by a member of the COA Domiciliary Care Team, in order to formulate the best possible care plan. There is ongoing monitoring and reassessment of people receiving services.

Domestic Assistance
COA arranges home support workers for short term (often after hospital care) to long term, for various levels of need. Usually the user pays, although financially disadvantaged people are eligible to apply for COA subsidy.

Information and referral
  • Information regarding all aspects of ageing
  • Referral to other relevant organisations, professionals, etc.
  • Help transitioning from own home to care environment
